Our Commitment:
Parkridge Centre Quality of Life Development Program is committed to protecting the privacy of the personal information of its employees, volunteers, donors, potential supporters and other stakeholders. We value the trust of those with whom we deal, and of the public, and recognize that maintaining this trust requires that we be transparent and accountable in how we treat the information that you choose to share with us.
During the course of our various projects and activities, we collect and use personal information. Anyone from whom we collect such information can expect that it will be carefully protected. Our privacy policy is based on 10 internationally recognized privacy principles which are detailed in a separate publication, entitled “Privacy Policy Principles,” available from the program.
We meet all legislative requirements with respect to privacy. We adhere to the privacy guidelines of the Canadian Centre for Philanthropy. We comply with the Donor Bill of Rights (see Page 52) developed by the American Association of Fund Raising Counsel, the Association for Healthcare Philanthropy, Council for Advancement and Support of Education and the Association of Fund Raising Professionals. We also comply with the e-Donor Bill of Rights adopted by the Association of Fundraising Professionals.
Defining Personal Information
Personal information is any information that can be used to distinguish, identify or contact a specific individual. This personal information can include an individual’s opinions or beliefs, as well as facts about, or related to the individual. Exceptions: business contact information and certain publicly available information, such as names, addresses and telephone numbers as published in telephone directories, are not considered personal information. Where an individual uses his or her home contact information as business contact information as well, we consider that the contact information provided is business contact information and is not therefore subject to protection as personal information.
Privacy Practices
The Quality of Life Development Program collects limited amounts of personal information about its donors and potential supporters, including name, title, address, telephone number, past donations, and other relevant contact information. This personal information is collected for the following purposes:
• to comply with Canada Revenue Agency requirements,
• to provide donors with periodic stewardship information,
• to provide donors with appropriate recognition by the Quality of Life Development Program and Parkridge Centre,
• to provide donors and potential supporters with information about Parkridge Centre and the Quality of Life Development Program activities, and
• to promote opportunities where donors and potential supporters may support the Quality of Life Development Program.
Personal information collected by the Quality of Life Development Program is kept in confidence and is only disclosed where necessary to meet the above purposes. For example, the program may disclose limited personal information to Parkridge Centre or the Saskatoon Health Region so that they may directly recognize a particular donation.
We collect, use and disclose personal information only for purposes that a reasonable person would consider appropriate in light of the circumstances. We do not rent, sell, or trade mailing lists or other personal information. Individuals also have the opportunity to opt out altogether of having us use their information except as required by law.
Release of Information
Without written permission from the donor, information regarding donations to the Parkridge Centre Quality of Life Development is strictly confidential. Only the Manager of the Quality of Life Development Program, the Executive Committee, Chair of the Steering Committee, others formally designated, may have access to such information, i.e., the Chair may authorize a treasurer to examine the records for accuracy or for audit purposes. Access to all program records is strictly on a “need to know” basis. Any and all requests for information from the records should be referred to the Manager of the Quality of Life Development Program. If the Manager is unsure whether to release the requested information, the request shall be referred to the Executive Committee for deliberation and resolution. Unauthorized release, use of dissemination of such information is grounds for immediate dismissal of employment or removal from the Steering Committee.
Website and Electronic Commerce (future)
We use password protocols and encryption software to protect personal and other information we receive when a donation is made online. Our software is routinely updated to maximize protection of such information.
Updating of Privacy Policy
We regularly review our privacy practices for our various activities, and update our privacy policy.
Openness about Personal Information Policies and Practices
The Program will make readily available to individuals specific information about its policies and practices relating to the management of personal information. The information made available will include:
• The name or title, and the contact information of the Chief Privacy Officer, who is accountable for Parkridge Centre and the Program’s privacy policies and practices, and to whom complaints or inquiries can be forwarded,
• The means of gaining access to personal information held by the Program
• A description of the type of personal information held by the Program, including a general account of its use and/or disclosure
• A copy of any brochures or other information that explains the policies, standards, or codes, etc.
